SMART is actively recruiting for a locum Emergency Medicine Physician to provide coverage for a client of ours in Kentucky.

• Shifts Available:  Day and Night
• Please provide days and times below.: 7a-7p 11a-11p 7p-7a
• Please provide the number of shifts that locums will cover per month.: 15
• What EMR system does this facility use?: Meditech

• What is the annual patient volume?: 43,500
• How many ED beds?: 22
• Will FM and/or IM trained clinicians be accepted:  Yes
• What is the patient volume per hour?: 1.8
• Are scribe services offered?: No
• EM experience : 3 + years
• Trauma designation Level: Level 3
• Who responds to Codes?: HM team
• Supervision Responsibilities?: APC

• Will temporary or emergency privileges be granted?: Yes
• What is the average time to obtain temporary privileges?: 45 days 
• Will the hospital release their application and process pending license?: No
• if not Board Certified, years of eligibility defined by facility?: 6 but depends on the certification
• Please select all required life support certifications: ACLS, ATLS, BLS, PALS
